Benefits Providers

As a courtesy to our patients, we will complete and give you guidance on the filing of insurance forms relative to your dental care.

It is important to remember that you are a partner with us in your oral health, and that treatment and care decisions should be made together with us, based on your actual needs.

A dental plan is a way of helping you cover the cost of dental care. However, your dental plan may not cover the full cost of the specific treatment you require.

By taking care of your dental health, you are making a long-term investment in your own well-being.

Medical Updates

Changes to your medical health are just as important to us as is your dental health. 

It is important for new patients to keep us informed about any issues regarding their overall health and what medications they are on. Kindly bring a list of any medications you are on to your first visit.   

Change of Appointments

Appointments are scheduled specifically for you. Our team will contact each of our patients prior to a scheduled appointment with a courtesy reminder. However, it is each patient’s ultimate responsibility to arrive for the scheduled appointment.

We require a minimum of 2 full business days notice to change and/or cancel appointments. If an appointment is missed, a missed appointment fee will apply.


It is the patient's responsibility to pay the fees for their dental procedures.  We accept the following methods of payment.
