Your First Visit
Please arrive about 10 minutes early for your first appointment so you have enough time to fill out any necessary forms.
*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.
Your dentist will begin by reviewing your medical and dental history and completing a comprehensive dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening and any required X-rays.
Based on the findings of your history and examination, your dentist will develop a long-term treatment plan to help improve and maintain your oral health. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.
As a new patient at our clinic, you are bound to have questions about our office, payments options, and more.
